Skilled Nursing Services

MyLife™ Home Health is a Medicare certified home health agency. Our dedicated professional staff works directly with the patient's physician to prepare a personalized plan of care that best fits the patients needs. Our skilled nurses and therapists (PT, OT, ST) visit patients on a regular basis, usually for a period of 60 days, to provide ongoing care, guidance and support and to help each patient meet or exceed recovery goals.

MyLife skilled clinicians can provide:

MyLife Home Nursing provides customized, private duty services for medically complex, technology-dependent adults and children.

• RN/LPN hourly shifts in-home
• RN case management for coordination of all services
• Transitional Care to assist with coordination of services from hospital to home, including the development of a plan of care, home environmental evaluation and set-up of supplies and DME.
• Pediatric Specialty
• Trach/Ventilation Care
• Gastronomy care and G-Tube feeding
