Home Health Marketer/Nurse Liasons

Must have proven track record in obtaining Medicare referrals from valid referral sources. LPN or RN license is required!  He/she will represent Mylife and its services in a competent professional and responsive manner and maintain standards of high quality customer service in compliance with federal and state regulations and guidelines.

QUALIFICATIONS

  • Current license as an RN or LPN in the state(s) of practice.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
  • Strong communication and interpersonal skills

Home Health Registered Nurse
Provides services to clients in accordance with the State Nurse Practice Act, Agency policy and accepted standards of practice.

The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.

QUALIFICATIONS

  • Graduate of an accredited school of professional nursing. BSN, PHN preferred.
  • Current license as a Registered Nurse in the state(s) of practice.
  • Minimum of one year experience in an acute care setting or equivalent experience.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
  • Current CPR certification.
  • Strong communication and interpersonal skills

Private Duty Registered Nurse
Provides services to clients in accordance with the IL Nurse Practice Act., MyLife policy and accepted professional standards of practice.

The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.

QUALIFICATIONS

  • Graduate of an accredited school of professional nursing
  • Current license as a Registered Nurse in good standing with the state of Illinois.
  • Minimum of one year experience in an acute care setting or equivalent experience.
  • Previous experience and a strong desire to work with children and families if seeking a pediatric assignment.
  • Current CPR certification.
  • Strong communication and interpersonal skills

PHYSICAL THERAPIST
Reports to: Director of Professional Services/Therapy Supervisor
Duties/Responsibilities:
Clinical Functions

  1. Plan and prepare a written plan of care based on the evaluation of available patient data & revise as necessary.
  2. Perform patient tests, measurements, and evaluations, and records and evaluate findings to aid in establishing or revising specifics of treatment programs
  3. Provide physical therapy services on an intermittent basis to home health patients
  4. Administer manual therapeutic exercises to improve or maintain muscle function
  5. Instruct, motivate and assist patient in non-manual exercises and functional activities
  6. Observe, record and report to the physician or podiatrist the patient’s treatment, response and progress
  7. Prepare clinical and progress notes for the clinical record

Supervisory Functions

  1. Perform required supervision of the Physical Therapy Assistant
    1. The registered therapist will be available by telephone to the assistant at all times while the assistant is treating patients
    2. On-site supervision will occur every 4-6 visits
    3. The supervisory visit will include a complete on-site functional assessment, an on-site review of activities with appropriate revision of the plan of care, and an assessment of the utilization of outside resources
    4. The supervisory visits can occur either when the assistant is present to observe and assist or when the assistant is absent to assess relationships and determine whether goals are being met
  2. Instruct other health team personnel including, when  appropriate, home health aides and family members in certain phases of physical therapy with which they may work with the patient
  3. Instruct patient and family in total physical therapy program

PHYSICAL THERAPIST ASSISTANT
Reports to: Director of Professional Services/Physical Therapist
Duties/Responsibilities:

Agency Functions

  1. Adhere to Agency policies, procedures, regulations, standards, and code of ethics
  2. Participate in patient care conferences as requested
  3. Participate in in-services as requested
